Transport of human remains

TRANSPORT OF THE HUMAN REMAINS TO REPUBLIC OF SERBIA

Transport of human remains, either decedent's body or cremated ashes, to Republic of Serbia requires a permit called SPROVODNICA;

The following documents and procedures are required:

1. The form sprovodnica should be filled out by a family member

2. A valid Serbian passport belonging to the deceased. If deceased does not have Serbian passport, a valid Canadian passport or other photo ID can be accepted.

3. Original Death Certificate notarized with an Apostille stamp at the Official Documents Services (777 Bay Street, Toronto, ON M5B 2H7, Phone: 416-325-8416 )

4. Cause of death issued by the local health authority.

5. Burial-transit permit for transport of the human remains to Republic of Serbia, issued by Canadian authorities .

6. Certificate, issued by a Funeral home stating that the decedent's body was prepared for transport according to the international regulations.

7. A statement from the Funeral home that the deceased was identified (name of the person who identified the deceased) and that the coffin (urn) contains nothing but the body / ashes of the deceased.

8. Certificate of the burial place provided by the cemetery in Republic of Serbia, if the deceased is not a Citizen of the Republic of Serbia.

Please note:

Numbers 4, 5 and 6 do not refer to transfer of the urn. In that case it is necessary to provide the Certificate of Cremation.

 

In addition:

  • Payment: Money order or certified check in the amount of 7 CAD (for the permit called SPROVODNICA) payable to Consulate General of the Republic of Serbia or by debit card at the Consulate
